Therapy is all about having a friend. Some good therapists are great listeners and can make the patient feel like they have a best friend. It works well because most therapists are trained in listening and know how to interact with patients.

In real life, people don't want to be concerned. You can talk to someone about your problems for a minute, but if you delve deeper most people turn off, even those close to you. A therapist is different because they are trained to help you get to the root of the problem and take care of it. A lot of patients just feel happier knowing they have another friend (the therapist) to talk with.

Depression therapy involves finding a good therapist. The therapist will often put you on some antidepressants and also use cognitive therapy in conjunction. As a patient, you can refuse antidepressants. There is conflicting evidence as to how effective antidepressants are in comparison with therapy. It is too easy for a therapist to just give you a drug and then do nothing. Most people are depressed for reasons that are cognitive and a drug can't fix that. It can only medicate it indefinitely.

The reality of taking drugs or not will depend on how severe the condition is and how comfortable you are with the side effects. Most people think that antidepressants are over prescribed and not effective enough. Depression statistics show that around 30% of people on antidepressants are helped by the drug. I would imagine that around 30% of placebo patients feel happier with that too.
